Home From Hospital Care provides support for Sandwell residents who have recently been discharged from City hospital and Birmingham residents who have been discharged from Birmingham hospitals to attend medical appointments or collect prescriptions. This service offers transport and company for people who are unable to attend their appointments independently. This service supports people to maintain their health, reduces anxiety and provides much needed companionship for people who may be isolated.
