Home From Hospital Care provide food parcels on discharge for Sandwell residents leaving City Hospital and Birmingham residents leaving Birmingham hospitals. The food parcel supports people leaving hospital who may have been admitted for some time and would be going home to empty cupboards and an empty fridge. The parcel is substantial and provides food for several days. It can be delivered to the patient on the ward on the day of discharge. 24 hours notice is required where possible.
