Home From Hospital Care provides support with shopping for those who are unable to do this independently. This can be taking people to the supermarket or doing their shopping and delivering it to them at home. This service is for Sandwell residents recently discharged from City Hospital and for all Birmingham residents discharged from Birmingham hospitals. The service is fro six weeks following discharge from hospital and is provided by aftercare coordinators or volunteers. Support is provided to make arrangements for shopping support following the end of the six week service such as setting up meal delivery services or telephone or on-line shopping.
