Home From Hospital Care provides befriending services to Sandwell residents who have recently been discharged from City Hopsital. The befriending service is for six weeks following discharge. The adult will be visited at least once a week by a project coordinator or a volunteer. The befriender will visit the adult at home and provide emotional support, company and companionship. Assist with some practical tasks and can give advice, information and signposting to other local services which may be helpful. The befriending service can also support people to access local community groups and resources. Befriending supports people to stay in their own home for longer, reduces the likelihood of readmission to hospital and can improve mental health and emotional wellbeing.
