The Healthy Communities Collaborative Team (HCC) is part of the Pathfinder Healthcare Development CIC and work across the West Midlands.
The team, alongside volunteers, engage individuals using an innovative health assessment tool; this is used to identify individuals who are at risk of presenting early
signs and symptoms of long terms conditions, and using clinical pathways refer directly to further clinical intervention. The team hand out information leaflets, whilst supporting and encouraging people to take practical steps to improve their lifestyles.
