Home From Hospital Care offers support to Sandwell residents who have recently been discharged from City Hospital and Birmingham residents who have been discharged from Birmingham hospitals with benefits checks and support around claiming benefits. The service can also support with information and advice, referrals to other agencies and signposting and support with correspondence and bills. This service helps to reduce anxiety and enhances wellbeing, supporting people to remain at home and reducing the likelihood of readmission to hospital.
